Family Based Care Management

Our Programs
DelgraciaCorp's programs are built on a two-generational, family preservation model that recognizes the well-being of parents and children are deeply connected. We support young parents with children who are transitioning from foster care, homelessness, justice involvement, or other systems of care by addressing the social, emotional, economic, and parenting challenges that place families at risk of continued system involvement. Rather than responding only during times of crisis, our programs integrate family-based care management, mental and behavioral health services, peer support, leadership development, and community-based partnerships to strengthen protective factors and promote long-term family stability. By combining prevention, healing, and practical support in one coordinated model, DelgraciaCorp helps young families build self-sufficiency, break cycles of intergenerational trauma, and thrive within their own communities.
DelgraciaCorp provides trauma-informed, family-centered care through long-term relationship building, 24/7 access to a care manager, brief crisis intervention, emergency financial assistance, and coordinated mental and behavioral health services.
